We have openings for home health aides to join our growing team. You will be tasked with helping clients with personal hygiene, housekeeping, and other daily needs as required by the client. To excel in this role you should have an interest in health care and the ability to work with limited supervision keeping the clients needs as your top priority.

Responsibilities:

*Preparing light meals and assisting in feeding clients if necessary; *Help clients with personal hygiene, dressing, bathing, and other daily tasks.

*Perform basic health care services for clients including checking vital signs

*Help with general light housekeeping.

*Observe and report on client condition.

*Keep up with in-service trainings.

Requirements for positions:

*High school graduate, OR has completed a GED or equivalent

*One year experience as an institutional nursing aide or in-home Home Health Aide within the past two years.

*Reliable transportation with Drivers' Insurance and valid State-issued Drivers license.

*Must be at least 18 years old.

*Must be able to pass a criminal background check.

*Must be able to provide references.

*Ability to work with limited supervision.

We have openings for CNA/STNAs to join our growing team. Were looking for Certified Nursing Assistants and State Tested Nursing Assistants to provide basic care to our clients and help them with daily activities that they might have trouble doing on their own, such as bathing and getting dressed.

Responsibilities:

*Preparing light meals and assisting in feeding clients if necessary;

*Help clients with personal hygiene, dressing, bathing, and other daily tasks.

*Performing range of motion and other simple procedures as an extensional therapy service as ordered;

*Assisting in ambulation and exercise according to the plan of care;

*Taking and recording oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure when ordered;

*Assisting client in the self-administration of medication;

*Reporting clients condition and significant changes to the assigned nurse;

*Providing personal care, including baths, personal hygiene, changing bed linen, dressing/undressing, toileting activities, and light housekeeping;

* Assisting with patient’s laundry if needed;

*Following all privacy policies, Infection Control protocols, and maintaining the confidentiality of protected healthcare information (PHI);

*Adhering to the organization’s documentation and care procedures and standards of personal and professional conduct;

*Meeting safety needs of clients and using equipment safely and properly (foot stools, side rails, etc.);

*Informs licensed nurse of any changes in client condition/needs, of client complaints, and of client/significant other educational needs;

*Performs other duties as assigned;

*Contributes to safety and quality care by communicating to other team members.

PRN Position Only:

We have openings for LPNs and RNs to join our growing team to complete head to toe assessments.

Responsibilities:

*Assessing clients and determine care needs and level of care needed.

*Creating care/ Service Plans in coordination with the client or responsible party, and the client’s physician and other healthcare providers.

*Performing regular assessments and observations.

*Researching and recommending ways to improve client’s quality of life.

*Preserving the privacy/ confidentiality of client information and agency matters.

*Maintaining accurate time records and submitting required records on time.

*Responding to emergencies and participating in weekend and holiday shifts/schedules.

*Reporting all suspected or witnessed cases of abuse and serious incidents to the relevant bodies as directed by rules and regulations.

*Assisting the Administrator to make decisions related to care provisions and services.

*Complying with and ensuring compliance with agency policies and procedures.

Requirements for positions:
